A liability waiver is a legal document that is designed to protect individuals or organizations from liability for injuries or damages that may occur during a particular activity or event. This form is required for admittance and participation in any of Reality Check, Inc.'s events, programs and facilities.
A medical form is a document used by Reality Check staff to collect important medical information about teens participating in our programs.
